UPDATING INFORMATION IN FINAL FORMS IS REQUIRED EVERY YEAR AND THIS MUST BE DONE BEFORE ANY PARTICIPATION, INCLUDING WORKOUTS OR TRYOUTS. 

 Parents and prospective athletes,

 Red Oak Middle School offers 14 different sports programs throughout the year.  Our teams represent the school and community in the Tri-County Athletic Conference.  Each season (Fall, Winter and Spring), our teams will begin with tryouts and there is a process that is followed for those tryouts.  Each athlete must complete several forms, one time each year, and obtain a current physical to be located in the Athletic Director's office. Some fall sports tryouts will begin during the summer months and others as soon as school starts.  Therefore, it is a must that athletes have the following information and complete their portion of signing up for a team to be able to try out.

Red Oak Athletic Department

Red Oak Middle School Athletic Department is partnered with FinalForms, an online forms and data management service. 

FinalForms allows you to complete and sign athletic participation forms for your students. FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

We are asking that ALL parents of athletes use FinalForms.
